Our provider will conduct an OAE (Otoacoustic Emissions) hearing screening in the comfort of our beautiful office**. OAE hearing screenings evaluate the inner ear’s response to sound. The test is especially helpful for infants, children and adults who may not be able to respond to behavioral hearing tests because of their age or ability.

Our provider can perform basic laboratory testing during your house call, or office visit, including rapid influenza, rapid RSV, rapid strep, rapid COVID-19, finger-stick glucose, finger-stick hemoglobin, urine pregnancy, stool blood testing and urinalysis testing.

***All lab testing must be accompanied by a house call or office visit***

Our providers can complete many medical forms and certifications often required throughout a little one’s childhood. Ms. Julie can also complete certain forms for parents, when appropriate.

ESTABLISHED PATIENTS

$0 Fee

Completion of standard form(s) for an established patient, who has completed a Bluebird Pediatrics wellness visit within the past 3 months (if under 12 months of age), 6 months (if between 12 and 24 months of age) and 12 months (if over 24 months of age).

$50 Fee

Completion of personalized/complex form(s) and letters for an established patient, who has completed a Bluebird Pediatrics wellness visit within the past 3 months (if under 12 months of age), 6 months (if between 12 and 24 months of age) and 12 months (if over 24 months of age).

$50-$100 Fee

Completion of standard form(s) and/or personalized/complex form(s) and letters for an established patient, who has not completed a Bluebird Pediatrics wellness visit within the past 3 months (if under 12 months of age), 6 months (if between 12 and 24 months of age) and 12 months (if over 24 months of age).

NEW PATIENTS

$0 Fee

Completion of standard form(s) for a new patient, when paired with a new-patient office visit, house call or virtual visit

$50 Fee

Completion of personalized/complex form(s) and letters for a new patient, when paired with a new-patient office visit, house call or virtual visit.

When scheduling, please confirm with our office that the forms/certifications you desire are a service our clinic provides. Provider and medical discretion apply.

Our providers are extremely experienced and educated experts in both pediatric and family medicine. They are also extremely valuable advocates and are always willing to provide their expert recommendations when needed.
